| 75 mg | 75 (75) | 12 hourly | IV | Treatment of overdaosage of folic acid antagonists: This dose is followed by 12 mg IM every 6 hours for 4 doses. |
| 15 mg | 15 (15) | 24 hourly | PO | Treatment of megaloblastic anemia. Dose should not exceed 1 mg daily when given IM. |
